Challenges

Lady Willingdon Hospital is committed to providing essential healthcare in a challenging environment. But now we need your support to ensure these vital services remain available to the people of Manali. Here are some of the key challenges we are facing.

Aging Infrastructure and Space Constraints

  • Our hospital building, constructed in the 1940s from wood and stone, is showing its age.
  • The aging infrastructure presents risks such as fire and infections, although our infection rates remain mercifully low.
  • Maintenance of the older building is costly, and there are limitations on expansion and modification.
  • Space is a major concern. Our outpatient waiting room, designed for 50-70 patients, often accommodates over 300, leading to overcrowding.
  • The antenatal waiting room is also cramped, with pregnant women facing discomfort due to limited seating.

Patient Flow and Facility Design

  • The current layout of the hospital creates challenges for patient flow. For example, the operating theatre’s location across the courtyard from the wards requires patients to be transported outside, exposed to the elements.
  • The absence of a ramp to the second floor necessitates patients being carried up the steps.
  • The layout of services within the hospital can be confusing, making it difficult for patients to navigate, despite the hospital’s small footprint.

Limited Specialist Care

  • While India’s doctor-patient ratio is improving, access to specialist care in remote regions like Manali remains limited.
  • Specialists tend to concentrate in cities, leaving rural areas underserved.
  • Consequently, our hospital is often called upon to provide complex care typically found in tertiary settings, addressing critical needs due to the lack of alternatives.
  • We are able to provide this high level of care safely because of the dedication of our staff.

Cost of Care and Financial Sustainability

  • A significant portion (80%) of healthcare expenses is paid out-of-pocket by patients, creating a financial burden.
  • Government health insurance schemes offer some relief, but their coverage is not yet universal.
  • Our hospital has had to opt out of some government schemes due to non-payment of dues, despite the government’s promotion of these schemes.
  • Demonstrating our commitment to care for all, our hospital allocates 30% of its earnings annually to support patients who cannot afford treatment.
  • Maintaining a balance between financial self-sustainability and providing care for the poor is a constant challenge, one we have navigated for decades.

Heating and Climate Control

  • Uneven heating in the wards and harsh winter conditions create discomfort for patients and difficulties in maintaining climate control.
  • While room heaters and blowers are used, they are not an ideal solution.

Terrain and Accessibility

  • Manali’s location at 6,500 feet, near the 13,500-foot Rohtang Pass, presents unique geographical challenges.
  • The altitude, combined with the rugged terrain and harsh weather, makes access to healthcare difficult and can delay timely treatment, especially for those in remote areas.
  • The sparse population in the hill country adds to the challenge. While our hospital is one of the few equipped to handle a wide range of medical needs, reaching us, particularly during winter, is a significant hurdle for many.
